Conwy & Denbighshire Adult Mental Health and Social Care Partnership
The Partnership, which was formed in July 2005, aims to provide seamless services, holistic care and support for mental health service users and carers.
One of its objectives is to create a more powerful voice for users and carers by their involvement in planning, commissioning, delivery and monitoring of services.
A better, more joined-up approach, aims to reduce unnecessary duplication of services, with health and social care staff working together.
It is a PARTNERSHIP between:
